Pre-Employment Expense:

* The Woodlands Assisted Living Residence is required by the state to make sure that all new employees have a physical completed in order to meet State Regulations. The Woodlands will pay for a physical examination that is performed by U.S HealthWorks.

Should the employee resign from their position or be terminated within the first 90 days of employment, the employee will have a payroll deduction from their final paycheck for this expense. The cost of the examination is $83.00.

I understand that I will have this expense deducted out of my final paycheck if I resign or am terminated with the first 90 days of employment.

I also understand that I have the option of going to my own medical physician and have them perform the physical at my own expense.

I also understand that the background check fee will be deducted from my last paycheck in the amount of $37.00 for Ohio BCI check and/or %50.00 for the FBI BCI check if I should resign or am terminated within the first 90 days of employment.
